N.J. Council on Developmental Disabilities (DDC), 2003
(The DDC became its own administering agency as of 4/28/88. Its activities are independent of any other State entity, with the exception of fiscal account management and contract bidding, where it follows regulations from the NJ Department of Human Services or the NJ Department of the Treasury.)
Recipients of case management services have free choice of providers of case management and the providers of other medical care.

Early Intervention
Developmental Disabilities include infants and young children. An individual from birth to age 9, inclusive, who has a substantial developmental delay, specific congenital or acquired condition, may be considered to have a developmental disability if the individual, without services and supports, has a high probability of a limitation in self-care, receptive or expressive language, learning, mobility, self-direction, capacity for independent living, economic self-sufficiency, and reflects the need for individually planned and coordinated services.

I.D.E.A. Goal
All children in NJ with DD will receive a free appropriate education. Objectives include an increased understanding among the various constituents about the changes in and the implementation of federal and state education laws and regulations.

Goal: All transitions of students with disabilities in their educational career are smooth and seamless with positive, inclusive outcomes. Objectives include perceived and real barriers facing the various stakeholders at various points in the educational continuum.

Quality Assurance
Goal: People with developmental and other disabilities in NJ receive high quality supports and services.
Goal: People with developmental and other disabilities have a voice in local and/or statewide issues and concerns affecting their lives in the community.

Cross Cutting
Goal: People with developmental and other disabilities and their families are active participants in policy-making, educational and advocacy activities.

Case Management Services
Case Management services are targeted to Medicaid eligible infants and toddlers (birth through age 2) with disabilities who meet the eligibility criteria for services under Part C of I.D.E.A. Early Intervention services are available for children from birth up to their third birthday who have a developmental delay of 25% or more in two or more areas or a delay of 33% or more in; cognitive, physical (including vision and hearing), communication, social or emotional, or adaptive. Children from 0-2 who have a diagnosed physical or mental condition which has a high probability of resulting in developmental delay are also eligible.

Case management includes:
  1. Coordinating evaluations/assessments
  2. Facilitating/participating/evaluating IFSP development
  3. Assisting in identifying services/service providers
  4. Coordinating/monitoring delivery of available services
  5. Giving information on available advocacy services
  6. Facilitating transition
